information you submit through this form will be kept confidential and used only in ways that align
with our mission to inspire, create, and educate.
Your story will never be shared publicly, adapted into a song, or used in any educational or
promotional materials without your explicit consent.
We take reasonable measures to protect your information. However, please understand that no online
system can be guaranteed 100% secure.
Consent & Use of Story
By submitting your story, you acknowledge and agree to the following:
- You are choosing to share your story voluntarily.
- You understand that Hope by Song is a creative and educational nonprofit program.
- You grant Hope by Song permission to review your submission internally.
- You understand that your story may or may not be selected for further creative development.
If your story is selected: – You will be contacted before any song is created or shared. – You will
have the opportunity to discuss how your story is represented. – You may choose whether your story
is shared anonymously or with attribution.
You may withdraw your consent at any time before public use by contacting Hope by Song.
Important Acknowledgment
Hope by Song is not a counseling or crisis service. While we believe in the healing power of music
and storytelling, this program does not replace professional mental health care.
